Credit Application

To establish credit terms with please provide the following:

* Company Information:  Legal business name, address, phone, fax, business entity, year established, type of business, Federal tax ID.

* Bank Information:  Bank name, bank address, bank phone, bank contact name, bank account number

* Three Trade References:  Name, address, phone, contact

* Credit reference letters from 2-3 suppliers.

* Credit reference letter from company bank.

Resale certificate if resell of products.

We realize this may be a lot of information, but typically conducts business via pre-paid invoice using credit card or bank transfers. Since we have not conducted business under net terms to date, we will need the information above to establish credit terms on account. Once established we are quite flexible with credit terms. 

All sales are subject to the ASC Standard Terms and Conditions of Sale.
